New Patients 

Welcome to Valley Arthritis Care, LLC. We look forward to meeting you. On your first visit, we will ask you to complete our new patient packet forms. You can find these forms under resources, fill them out and print them prior to your visit. Plan to arrive 20 to 30 minutes prior to your scheduled appointment time. We will verify your insurance and register your information in our computer system. If your insurance requires a referral, please make sure this has been issued by your Primary Care Physician and bring this referral with you to your visit. Your insurance will not allow us to evaluate you without a referral on the day of your visit. If you do not have this information on the day of your visit, your appointment will be rescheduled for another day. As a courtesy to our patients we confirm all appointments 1-2 working days prior to the appointment date. If you are unable to keep your scheduled appointment, please notify our office 48 hours in advance. This courtesy allows us to be of service to other patients. If you arrive late or do not have the following information, your appointment may be rescheduled. If your insurance changes prior to your visit, please contact us and update us with the new insurance information. Prompt notification to our staff will ensure accurate billing to the correct insurance carrier. You will also need to obtain a new referral from your new insurance carrier.  

Please bring the following information to your appointment: 

  • Insurance Card(s)

  • Valid Photo ID

  • List of Medications (To fill out your health history form)

  • Your Referral 

  • Your Co-Payment and/or Deductible 

  • Any Pertinent Medical Records, Labs, X-Rays, MRI's. We do not need the actual films, just the reports. This will help us not send you for duplicated testing. We will copy what we will need and you can keep your originals.
